So this seems to be caused by Ollama, is inconsistent, and may have been fixed in a newer version. (I may have accidentally been stuck on an older version because their updater doesn't work correctly.)

I am resolving it for now by displaying an error when it occurs and skipping the affected file. Since it is an inconsistent bug, re-running the script should resolve it, and if it occurs multiple times, multiple re-runs will fix it.
